Determining core versus noncore privileges

For organizations moving toward the development of a criteria-based core privileging system, determining whether a certain procedure or privilege is core or special/noncore can be problematic. Core are those procedures or privileges that any well-trained physician within a particular specialty or subspecialty should be competent to perform upon completion of postgraduate training.

Ariya Ouskouian is accused of visiting two California hospitals between April and June and posing as a physician on several occasions.  Authorities allege that Ouskouian cleared security at Children’s Hospital of Orange County by saying he lost his hospital ID badge and asking for a temporary one.

New procedure/technology policy

It is common for physicians to come to work one day saying they want to perform the latest and greatest procedure or use a new technology. If your organization does not currently support the procedure/ technology and does not have privileging criteria for it, you need to take a step back and determine next steps. Having a policy in place to deal with any new procedures/technologies will help guide this process in a consistent manner.

Determine practitioner's past experience with privileges

If you receive an application from a practitioner requesting privileges that he or she did not hold at his or her previous hospital, don’t deny the request based on that fact alone. Instead, ask the practitioner questions such as the following.
