CMS Releases The Money Follows the Person: Updated MFP Grant Recipient Transitions Report

Today, CMS released the Money Follows the Person (MFP): Updated MFP Grant Recipient Transitions as of December 31, 2021 Report. The report relies primarily on self-reported data provided through MFP grant recipients’ semi-annual progress reports from 2017 – 2021. The data reports summarize progress toward the annual goals of transitioning eligible people to the community and increasing Medicaid support for community-based long-term services and supports (LTSS). Additionally, the reports help track progress and challenges encountered in all dimensions of the program.
