Eliminate "other" on privilege forms

Communication with new applicants regarding their request for clinical privileges will help clarify what may be required on the part of the applicant (e.g., additional documentation specific to certain privileges and what that documentation needs to be). It is also important to be sure that someone is not requesting a privilege/procedure that is currently not offered. This can be avoided by eliminating an “other” option on privileges forms.
