Analytics, Care Management, and Clinic Models to Reduce High Utilization

When Intermountain Healthcare discovered that 5% of its patients were consuming 51% of healthcare costs, it decided to address the issue. By tracking and analyzing these “hotspotters,” Intermountain was able to identify the common chronic conditions the patients shared, as well as a common problem: a lack of care coordination.

In crafting a solution, Intermountain took a three-fold approach: a new strategy for care coordination of complex patients; the creation of a comprehensive care clinic to deliver a “medical home on steroids” level of care to this key population; and a community-based model that reaches hotspotters at home. Patients who had been visiting the ER two times a month are now going three or four months between visits.
