28 June 2022 Carlsberg has been fined 3 million after a contractor died and another was seriously injured following an ammonia gas leak at one of its breweries. 21 June 2022 Sergeant Rachel Fisk died in 2021 while taking part in a parachuting exercise during an RAF training event. HSE will now lead the investigation.
