Neighbors Describe Shocking Scenes After Scaffolding Collapse in Chicago Injures 2 Workers City crews are trying to figure out what led to a scaffolding collapse in East Garfield Park that left two people seriously hurt, and witnesses are describing the chaotic scene. The collapse occurred Monday afternoon at a building being renovated in the 3300 block of West Warren. Two people were hospitalized, and mangled debris remained strewn all over the yard as investigators began their work. There were a lot of witnesses to the incident, with most describing the loud sounds of the scaffolding falling. One neighbor even watched it all fall down from the front window of her home. "I heard a huge, like almost describe it as an explosion. Just a huge cacophony of metal and glass," said neighbor TIffany Smith. Smith witnessed the collapse through her window. The residents were out of the home at the time of the fire, and the homeowners told crews that there were ten dogs inside the home. MFD says firefighters were able to locate several of the dogs, all of whom were found dead. An official count of the deceased dogs is unknown at this time. Chillicothe Community Fire Protection District and West Peoria Fire Protection District assisted with overhaul and fire suppression. The fire was contained in the middle section of the building. Six building units suffered flame and smoke damage. One person was injured and transported to an area hospital. WMBD-TV CBS 31 PEORIA Fatal fire at storage facility in East Alton One person died in an early morning storage unit fire in East Alton, Illinois. According to East Alton Police Chief Scott Golike, crews got the got call about the fire around 1:25 a.m. on the 600 block of West St. Louis Avenue, as it took firefighters nearly an hour to put the fire out. Upon arrival, the East Alton Police Department initially found a person dead in a storage unit. The identity of the victim has not been revealed, and the cause of the fire is still unknown. LOUIS Peoria Fire Division Chief: ATF ‘instrumental' to Planned Parenthood arson investigation Peoria Fire Department leaders are reflecting on the multi-agency federal investigation into the Planned Parenthood Peoria Health Center arson in January. Tyler Massengill, 32, of Chillicothe pleaded guilty in federal court on Feb. 17. He was arrested on January 24, just nine days after the January 15 arson. "Because of the nature of this building, the fact that it was a Planned Parenthood building, we knew there would be some federal action that needed to be taken…We were able to tap into those federal resources. Having those federal resources jump into the investigation and become part of this particular instance, was very helpful. It really put a lot of resources into this particular incident," said Nate Rice, division chief of investigations at Peoria Fire Department. WMBD-TV CBS 31 PEORIA Illinois Department of Agriculture's Grain Bin Safety week stresses farmer safety practices The Illinois Department of Agriculture (IDOA) and the Illinois State Fire Marshal (OSFM) encourage Illinois farmers to review safety practices while working in and around grain bins during Grain Bin Safety Week, which runs February 19 through 25. According to researchers at Purdue University, more than 900 cases of grain engulfment have been reported with a fatality rate of 62% in the past 50 years ending in 2010. In 2010, at least 26 U.S. workers were killed in grain engulfment accidents, which is the highest number on record. In only four seconds, a full-grown adult can sink to their knees in flowing grain. In only 20 seconds, a full-grown adult can be completely buried in flowing grain. Suffocation from engulfment is the leading cause of death in grain bins. Man killed in explosion at Ohio manufacturing plant; at least 13 others taken to hospital VIDEOS: One person is dead following an explosion at an Oakwood Village manufacturing plant Monday afternoon, the Cuyahoga County Medical Examiner's Office confirms. Officials are expected to provide new details during a 2 p.m. press conference today, which we will stream live. Multiple crews from across Greater Cleveland and Akron responded to the I. Schumann & Co. metal plant on Alexander Road, near Bedford, where the blast led to a fire just after 2:15 p.m. Oakwood Fire Capt. Brian DiRocco says at least 13 others were taken to local hospitals, with several suffering burn injuries. The deceased victim has been identified as 46-year-old North Ridgeville native Steve Mullins, and sources tell WKYC's Lydia Esparra he was a maintenance worker at the plant. Brian DiRocco Tesla driver killed after barreling into ladder truck blocking accident scene on California freeway PHOTOS: Clean up for a crash on a northern California freeway turned deadly Saturday after a Tesla plowed into a fire truck that was shielding first responders who were working another accident. The driver of the Tesla Model S was killed when the car hit the truck that was parked on Interstate 680 around 4 a.m., according to the Contra Costa County Fire Protection District. A passenger was also critically injured. Assistant Chief Tracie Dutter said four firefighters who were in the ladder truck were treated for minor injuries. Multiple photos posted on the agency's Facebook page showed the Tesla with a crushed front end and the $1.4 million truck preparing to be towed. Dutter said the truck had its lights on and was parked diagonally in northbound lanes of the freeway to protect first responders who were working an earlier accident that did not result in any injuries. On the morning of Tuesday, Feb. 17, 1948, fire destroyed the 21-year-old Houck Field House on the State College campus. Not only was the field house the home of the Indians' basketball games, it also housed hoop competition for Cape Girardeau Central High School, College High School and the Sunday School League. In addition it served as the site for political rallies, patriotic assemblies, religious meetings, Scouting events, cooking schools and an untold number of other public gatherings. Built in 1927 through the efforts of then college president Dr. Joseph A. Serena, loss of the field house was significant for the college and the community. State Fire Marshal Timothy McLaughlin said the overhaul of the code has made Rhode Island "probably the safest state in the country" when it comes to fire safety. "When they did this in 2003, they did it for a reason," McLaughlin said.
