Start the new year off on the right foot: Restore proper coding query practices

Key performance indicators: Using data to strengthen denials management

Healthcare News: Fall 2021 hospital safety grade report released
The fall 2021 Leapfrog Hospital Safety Grade report has been released, assigning grades to 2,901 hospitals. The grades come from hospitals’ performance on over 30 evidence-based measures of patient safety. For the first time, a new grading factor for post-operative sepsis, blood leakage, and kidney injury were included in those measures.

ICD-10-CM codes from category E05.- (thyrotoxicosis) are combination codes that indicate the presence or absence of thyrotoxic crisis or storm. A thyrotoxic crisis (also called "thyroid storm") is a condition that may occur among individuals with hyperthyroidism and is marked by tachycardia, fever, and hypertension, which may or may not result in congestive heart failure and/or shock. Thyrotoxic crisis should only be coded when specified by the provider and cannot be assumed based upon other symptoms alone.
