The Office of Inspector General (OIG) recently performed a compliance audit of specific diagnosis codes billed by Anthem Community Insurance Company Inc. to CMS and found that the company submitted unsupported diagnosis codes for 123 of the 203 enrollee-years.

ICD-10-CM codes in category L89.- (pressure ulcer) are combination codes that do not require a second code to describe the stage. The sixth character of the code indicates the stage. Coders may choose the stage based on clinicians' documentation, but the physician must establish the type of ulcer.
