When reporting ICD-10-CM code Z48.3 (aftercare following surgery for neoplasm), use an additional code to identify the neoplasm. If it is unclear if the neoplasm has been eradicated or if there are plans to continue treatment for the neoplastic disease, assign the appropriate code from Chapter 2 of ICD-10-CM to indicate the neoplasm. If the documentation clearly indicates that the neoplasm has been eradicated, assign the appropriate code from ICD-10-CM code category Z85.- (personal history of malignant neoplasm) or Z86.- (personal history of certain other diseases).
