A peptic ulcer should be coded to ICD-10-CM code category K26 (duodenal ulcer) only when specified as duodenal or postpyloric. When documentation reports an ulcer as peptic and does not specify to one of these locations, assign a code from category K27 (peptic ulcer, site unspecified).
