Douglas Campbell, MD, FAAP, MHA Pediatricians may clinically agree about when a patient is in respiratory failure—they know it when they see it—but getting everyone to agree on an overarching set of clinical parameters for a documentation improvement program is far from easy. That means CDI professionals struggle to identify pertinent clinical indicators within the medical record to draft effective queries. It also means the clinical definitions that are applied may or may not equate to the appropriate ICD-10-CM codes.
