Regardless of time, you're still zoning out

 
Here're a few questions borne of an interesting problem that I've never thought of, since all of my coworkers are on the East Coast.
 
Remote and distributed teams: How do you handle time zone differences when working together?
 
Teams with members on both US coasts: When do you schedule team meetings? How do you coordinate all-team calls to accommodate the wide swath of time zones a distributed team could have?
 
What do you do in scenarios where only one team member is in a separate timezone? If you've been that one team member in a separate time zone, how did you cope?
