Peer review, quality, and patient safety: The medical executive committee’s role

Peer review, quality, and patient safety—the literature on these topics could fill a small library. However, the role of the medical executive committee (MEC) is not to become subject matter experts on these topics, but rather to effectively discharge their duties and responsibilities. Therefore, this article will provide some definitions, background, framework, and lists to give MEC members a high-level overview of the topic, so that they won’t embarrass themselves when talking about the subject.

Swiss cheese model and patient safety

A model frequently used to describe medical error is the “Swiss cheese model” described in the book Managing the Risks of Organizational Accidents by James T. Reason. The concept is simple: reduce the number of holes (errors) and increase the number of barriers (error prevention processes). If the holes in the cheese line up, an error reaches the patient, potentially causing harm. Keep in mind that as no person is perfect, neither is any barrier perfect. The concept is shown here.
