Anthem agrees to record-setting class action settlement for 2015 breach

Anthem, Inc., an Indianapolis-based insurer, agreed to pay a record-setting $115 million to settle a class action lawsuit filed over a 2015 breach that affected 78.8 million individuals.

"If you have a laceration that extends down the face inferiorly from the eye to the cheekbone, you would first code the repair of the eyeball, then the eyelid skin or muscles, and then the cheek. You would code all of them, to their associated depth (skin, muscle, etc.), even if the eyelid is 'beneath' the globe," says Erica E. Remer, MD, FACEP, CCDS.
