The principles of principal diagnosis selection

One of the primary difficulties in achieving uniformity of code assignment is that, in some circumstances, selecting the principal diagnosis is believed to be up to the individual coder or CDI specialist. Let’s take a closer look at the 2017 ICD-10-CM Official Guidelines for Coding and Reporting to understand whether this is really the case.
Most hospitals penalized in first five years of HRRP

CMS penalized more than half of hospitals participating in its Hospital Readmissions Reduction Program from fiscal year 2013 to 2017, according to a recent Health Affairs study. The average penalty increased from 0.29% in 2013 to 0.6% in 2017. Health Affairs referred to this as a modest penalty, bearing in mind that opportunities for penalization have increased since 2013.
