University sends prescription information on postcards

The University of Wisconsin-Madison is the second organization in recent months to run afoul of HIPAA by sending patients mailings that left protected health information visible.

"If we as coders are taking the easy route and are using unspecified codes, then the payers are not seeing the changes in conditions and may not necessarily see the medical necessity of their long-term care. As we start moving toward risk-based reimbursement and whole health, this is also going to start affecting reimbursement as well."

Q: It appears that CMS allows for HCPCS codes such as C1730 (catheter, electrophysiology, diagnostic, other than 3D mapping [19 or fewer electrodes]) to be associated with revenue code 272 (Supplies) or 278 (Implants). I searched for documentation form CMS and found vague references to both. Where we can find definitive documentation from CMS on which revenue code to associate with these HCPCS codes?
