These documents protect your organization as well as the entities that release information to your organization throughout the credentialing process. The length and format of attestations, acknowledgments, and release forms vary among organizations. Some are several pages long while others are just summaries. Either format is acceptable as long as several key elements are included as well as any state- or organization-specific requirements. Legal counsel should always review and approve any substantive changes to the information included in these documents. |

New Content: Members Only | Endovenous laser treatment - Procedure 215 Published 10/30/17 | Endovenous laser treatment (EVLT), also called endovenous laser ablation, is a minimally invasive procedure involving the use of a highly concentrated laser to repair varicose veins. Typically found in the legs, these gnarled, enlarged veins are caused by a faulty valve or weak vein wall. Although primarily a cosmetic concern for many people, varicose veins bulge and block blood flow, which can cause discomfort and lead to larger health problems (such as eczema, pigmentation, ulceration, and bleeding) if proper blood flow is not restored. |
