To mask or not to mask? (There’s no question)
Fred Pescatore Dear Reality Health Check Reader,

If you think the mask debate is long behind us…

Think again.

Well, I guess it was only a “controversy” for those who dismiss science—or selectively choose in which science to believe. Because masks work. They always have.

But you don’t just have to take my word for it…

A new study reveals how much lower COVID-19 transmission rates could have been IF mask mandates had been widely followed.

Nearly nine times lower

You might be wondering why I’m talking about masks in 2025. But please, bear with me.

Masks remain one of the simplest and most effective ways to prevent the spread of respiratory viruses—especially among our most vulnerable populations.

I’m talking about influenza, respiratory syncytial virus (RSV), the common cold, and yes—COVID-19.

But to really analyze the effectiveness of mask wearing, researchers pulled data from 7 million contacts from the U.K.’s COVID-19 app database.

Researchers used this data to develop a mathematical model of virus transmission. And then they focused on correlations between length of exposure and COVID-19 infection rates.

Ultimately, the model estimated that virus transmission would have been nearly nine times lower if all U.K. residents had simply worn a mask.

(While this is a mathematical model, it contributes to the growing evidence supporting the benefits of mask-wearing.)

But instead, the mask mandate was removed—and research suggests that virus transmission, hospitalizations, and death rates grew.

Let’s take a look…

It’s a public health initiative

A 2024 study found that mask use in Japan dropped from 97 percent in 2022 to 63 percent in 2023—potentially contributing to 3,500 additional deaths.

Moreover, a randomized clinical trial published in the British Medical Journal found that masks reduce respiratory symptoms. In fact, researchers landed on a relative risk reduction of 27 percent among mask-wearers compared to non-wearers.

And finally, did you know that widespread mask use likely contributed to the extinction of an entire flu strain—B/Yamagata—during the COVID-19 pandemic? (One less flu strain circulating is most definitely a positive.)

Yet, those facts often get overlooked.

Look, I realize this topic will always spark a debate. Everyone has their own opinion on whether universal masking or shutdowns were the right approach during the COVID-19 pandemic.

But the facts don’t lie. Those initiatives helped reduce spread and saved lives. But if you want some cut-and-dry advice, here’s mine…

I have a Master’s degree in public health, so this subject is dear to my heart. Communicating the importance of public health initiatives and persuading people to support them has always been a challenge.

Just remember that it’s crucial to rely on rational thinking over emotions. That’s why I’ve always advocated for mask-wearing. And I will remain dedicated to providing you with information on how masks protect you (and others) from all kinds of respiratory viruses—not just COVID-19.

Until next time,
Dr. Fred
