As defined by the World Health Organization (WHO) and recognized by the U.S. Department of Health and Human Services (HHS), social determinants of health (SDH) are the conditions in which people live, work, play, and age. SDH affect a wide range of health-related outcomes such as chronic conditions, preventable hospitalizations, morbidity, and mortality. But current analyses, predictive models, and prevention initiatives focus on addressing SDH at the population or zip code level thereby missing an individual patient’s needs.

 

During this session, Dr. John Showalter, Chief Product Officer for Jvion, will discuss new AI approaches to identifying individual clinical risk based on SDH data that then leads to more specific and tailored interventions. By the end of the session, attendees will be able to:

  • Define AI and its relevance for community-based risk predictions and interventions
  • List the shortcomings of current analytic approaches to population health management
  • Describe the data required to power an AI-enabled Socioeconomic Determinants of Health (SDH) solution
