To apply, you must be a licensed registered nurse, advanced practice registered nurse or nurse faculty member working full time at an eligible Critical Shortage Facility or an accredited school of nursing. Application Checklist - Proof of U.S citizenship, U.S. national, or lawful permanent resident - Upload a copy of yourU.S. birth certificate, an unexpired U.S. Passport ID or U.S. Passport Card, a Certificate of Citizenship, a Naturalization Certificate, or a Permanent Resident Card (Green Card). (Please note: the following are NOT acceptable: a driver's license, social security card, or nursing license).
- Authorization to Release Information Form -Download the form from the online application, fill it out, sign it, and upload it to the application.
- Transcripts -Request all transcript(s) from each school attended for all coursework related to the attainment of your nursing degree(s) with your name, institution's name, dates of attendance and courses. Unofficial transcripts are acceptable.
- Curriculum Vitae/Resume - Update and upload your resume/CV with your education, training, degrees, and a complete nursing employment history; include explanations for any gaps.
- Employment Verification Form - Send this form electronically through your application to the appropriate official or authorized point of contact at your critical shortage facility or nursing school to complete on your behalf.
- Loan Documentation -Review pages 15 and 24-25 of the Application and Program Guidance to learn which loans qualify and what information is required.
