One of the things we do with our interns is present patients for them to work with and manage. This is usually done by giving them information and letting them set a plan of action, and then we text back and forth as if the intern is the caregiver and either Steve or I is the patient. We have seen the positive results of this in the ability of the intern to have almost instantaneous communication with the patient.
